Need help deisnging simple content submission page

Hey guys, im a sophmore at the Unv. of Denver and am developing a website for a club sport at the school. We have a members page but its hard to add new members. I was wondering if anyone would be willing to help write up a php script to have a online submission page where the author could submit a photo and three diffrent fields of text, have that information stored in a database. Then have a page refrance that information on the members page. That info would also be put into an html style we have already arranged for the members info now. Thanks guys for your help!
