PHP Newbie - simple amend to create select list

To explain - I am trying to amend Coppermine Photo gallery - this for a school education charity -I've done some other picture field related changes But now I am totally beyond my experience, and stuck, as I need to have a select country drop down list in field user_profile3 . - so I attach the .zip file of the profile form. I know it is probably a very simple amend! - sorry!

I would be so grateful for any assistance on this matter as I have searched high and low for support in the coppermine forum on this
